  Welcome to the Break Through Your Set Point Personal Survey! The Break Through Your Set Point Program breaks down your lifestyle into five areas: eating, exercise, sleep, time management/stress, and motivation. By taking this survey, it will help determine the areas in which you are already doing great and what areas you can improve through the program.

The survey has 40 "yes" and "no" questions that will only take 5 minutes to complete. We do not collect the responses and you can take the survey anonymously. At the end of the survey, you'll get to print your responses to refer to at the end of the program.
